Office hours: 9 AM - 12 PM and 2 PM - 4 PM

The office is closed on weekends.

Dr. Acevedo cannot respond to phone calls during regular office hours.

Should your call be of an emergent nature, please notify the office staff and the call will be answered appropriately.

The preferred method of communication is by email. Please use the Contact Us page to send your message. This allows Dr. Acevedo to answer all messages and/or questions.

For lengthy topics of conversation or complicated questions better addressed in person, please schedule an office visit rather than an impromptu phone call.

We do not give out test results over the phone.

If testing is ordered, a follow up visit is required to thoroughly discuss the results and determine an appropriate plan of care. You should schedule this when the testing is completed.

Medication Refills: Please call us or contact your pharmacy and ask them to send us a request for refills.

DO NOT WAIT until you are out of medication to contact the office!

Please note that Dr. Acevedo's policy is that the patient must have a current office visit to receive medication refills. In most cases this is every three months. 

DMV Papers: The doctor will not fill out DMV papers until the patient is medically cleared to drive a vehicle. This may include testing and/or medication adjustments as needed. Cost for filling out DMV reports is $35 and must be paid prior to receipt of forms.

 *Please note that DMV papers must be mailed or hand    carried to the DMV by the patient.

I understand the office policies and realize that there are no exceptions.

Additional Services:

Prior Authorizations: It is not our responsibility to obtain prior authorization for any treatment. This includes any imaging, procedures or medications. 

Copy of Records: A patient is welcome to a copy of their records. If you need records, please contact the office and records will be prepared for you. Copy fees are $35 for charts.

Reports will be sent to the doctor that you designate on your intake form.

Life Insurance/Employer Medical Leave/Social Security papers, etc.: This type of paperwork is often extensive and requires a fee depending on the work involved.

Special Requests/ Formal letters: Fees for these types of letters is determined based on the time involved to complete the letter. With regard to these letters, prepayment is requested.

Fees for all additional services will be collected prior to service.
